Shawn K Wilson Dentistry accepts most dental insurance and will submit your claims to help you get the most from your dental benefits. We ask that you pay for any portion of the fee that you are responsible for at the time of treatment. We accept checks, Visa, MasterCard and Discover. We also offer third party financing through Care Credit.

Senior patients enjoy a 5 percent discount on treatment.

We know your time is limited. We want to use any time you have with us either in purposeful conversation or in treatment. We don’t want you to waste valuable time on paperwork!

To make your visit as efficient as possible, we have two options of completing your New Patient Forms.

By clicking on the New Patient Forms link below, our forms will open up in a browser (please ensure that your Adobe Acrobat Reader is installed or updated, you can download the FREE reader from here Adobe Acrobat FREE Reader ) and fill in the forms, then click “Submit” at the end of the document. When you “Submit” your filled out forms will then be emailed to our ladies at the front desk, or simply download and print your New Patient Forms and bring the completed form to your first visit.
